Registration for City Mosaic 2011

Eligible Age: 16 and above

| Program Fee: $120 per team of 4 (Fees include public transportation expenses and event T-shirt) |
| Early Bird: $80 per team of 4 (By April 25, 2011) |
| Registration closes on May 9, 2011 |
Online Registration: Each team is made up of FOUR members in order to register online. All four members' information must be filled out and program fee for whole team paid to complete registration.
After your payment, there will be a confirmation send out to every member on your team. Each member must sign the waiver form attached in the confirmation email. Please bring or fax them to Across U-hub by May 21, 2011 (Fax: 905-944-1950) in order to complete the registration process.

Refund Policy: There is no refund after May 9, 2011. For written cancellation request received on or before May 9, 2011, full refund less administration charge ($25 per team) will be returned.
